Who Can Enter:
Entrants must be current end-user clients and users of Avitar’s ORALscreen DRUGOMETER oral on-site screen for drugs of abuse, and be 21 years old or older.

How to Enter:
To enter, automatically be entered, review the on-line ORALscreen DRUGOMETER Tutorial, take the test, receive a passing score and a certification number. Send your name, address, daytime phone number, certification number, and request for certification to . Only one entry, per individual is permintted.

Winners will be selected by random drawing from among valid entries during the last business day of each month. All entries received that are illegible, or from ineligible entrants will be void.

The “Prize”:
One individual will be randomly selected each month, from certification received that month. Each winner will receive an iPod Shuffle. Winners are responsible for any additional expenses including, taxes and surcharges, if applicable. Blackout dates may apply. The iPod Shuffle is valued at $150.00. Winners will be notified by email and will be announced within one to two weeks of the random drawing. Except where prohibited by law, entry constitutes permission to use winners’ names, company names, and for online posting, and/or any advertising and publicity without additional compensation.

Prizes are not transferable for cash by winner. No substitution, transfer or assignment of prizes allowed by winner. Sponsor reserves the right to substitute a comparable prize of like or greater value, including cash, for any prize, for any reason.

The winners are responsible for paying all local, county, state, and federal taxes on prizes based on the estimated retail value of the prizes as set forth in these rules.

Odds of Winning; Miscellaneous:
Odds of winning depend on the number of entries received.

This drawing is subject to all applicable laws and regulations and is void where prohibited or restricted by law, including without limitation, in jurisdictions requiring registration, filing, trust account, or posting of a bond, or any other requirements or disclosures that are not expressly satisfied by these rules. Avitar accepts no responsibility or liability in connection with any injuries, losses, or damages of any kind caused by or resulting from the acceptance, possession, or use of any prize awarded hereunder.

By entering, participants represent that they are eligible, and agree (a) to be bound by these rules and the decisions of the judges, and (b) to release sponsor, its parent, subsidiaries, affiliates, directors, officers, employees, and agents from any and all liability for any injuries, losses, or damages of any kind caused by any prize or resulting from acceptance, possession, use, or misuse of any prize and (c) that entry constitutes an unlimited perpetual license for sponsor to use entry materials.
